Registration and Forms

Before you register:

1. Review all the information below and note all necessary forms. 

2. Gather all project information (title, abstract, category, etc.) and your advisor's contact information.

3. Set aside 10 - 20 minutes for the registration process. Registration must be completed in one sitting. The system will not allow you to save and return where you left off.

Please note that there are several demographic questions asked during registration which are part of our required grant funding reporting. Thank you for your cooperation.

4. Make sure your parent/gaurdian is present as you will need information about your health insurance policy, your physician's information, and you will need your parent to review and sign the waiver required for participation in the State Science Fair.

5. Please ensure that you have copies of your SRC paperwork for your records/reference & for Display & Safety

Follow the directions on the page to LOGIN to your account or CREATE an account if this is your first time on the page. 
Once you login, you will be taken to your User Profile page. Look on the lower right for the Event Registration section and click the link for the State Science Fair/JSHS.

For those not registered, there will be tickets sales for the awards ceremony.









Required Forms

Release and Waiver Forms

All State Science & Engineering Fair attendees who complete FULL REGISTRATIONS, including student participants, student observers, parents, teachers, and advisors, must submit Release and Waiver Forms.  Judges & Volunteers do not need to turn in waiver forms. Judges & Volunteers are to review waivers previous to registration and agree to all content on the waivers during their registration process. A la Carte attendees do not need to complete these forms.

Students, student observers, teachers, and parents need to fill out the appropriate set of waiver forms and email the completed form with your name and the word "waiver" in the subject line to Please email forms and do not send by mail. All forms must be received prior to the start of the competition. Students who do not have completed waiver forms in the office before the competition will not be able to compete. Release and Waiver Forms are not part of the SRC forms; they do not need to be displayed on your board.

Scientific Review Committee (SRC) Forms

SRC forms are distinct from the Release and Waiver Forms. Students must submit both sets of forms: Release and Waiver Forms and SRC Forms. SRC forms are determined based on the type of project a student completes. For more information about SRC forms, please visit the Project Rules and Guidelines page. 

Additional Forms

All students who win cash awards will need to fill out a W9 Form after the competition to collect their award. No students need to submit the form in advance of the competition.



-Refunds or cancellations must be requested in writing by email to by the final date for registration for the event for which you are registered.

-MAS will not permit refunds after the final registration date unless there are extenuating circumstances. Requests based on extenuating circumstances must be made in writing by email to the Program Director (Sara Gomez) before the program start date.

-Approved refunds will be issued no earlier than 30 days after the end of the program competition.

-MAS will not issue refunds for late fees.

Privacy Information

MAS will not share your personal or credit card information with any third parties.

Please be aware that by participating in a Minnesota Academy of Science sponsored event, you are automatically authorizing the Minnesota Academy of Science and its staff, agents, assignees, and affiliates to use, reproduce, and/or publish photographs and/or videos that include your image, likeness, or voice without compensation.

This material will become the property of the Minnesota Academy of Science and may be used online and in various print publications.
